There is no “CM” mint mark on Italian coins.

I think you mean designers initials CM. Yes, this coin has CM on it all years. You have one without them?

Yes

Here's is a picture of the official mint set from 2002. The mintage 150,000 matches the BU year line.

I can't find anything on different BU coins without the CM.

Me neither !!!

Clearly, this one has been in circulation for some time.  It looks 100% genuine.  I've had a look under the best magnification I own (mag x40) and there's no sign of any tampering.   When I found it I was simply comparing CM sizes, as per the Numista catalogue page, so you can imagine my surprise.  I've currently got it listed under the BU issue, but that's obviously incorrect.  That's why it's so strange that no one has found one of these before - the 2002 issue must be regularly checked.

Not sure how a random part of the design could be missing.  Any suggestions?

Has anyone got any suggestions as to why the CM initials are missing on this coin?  Is this a new coin that's not yet in the catalogue?

Most likely a grease filled die error.  When over a trillion are made this type of error is bound to occur.

In the case of: “grease filled die error” - if you have it in your hands you can see a bit different surface.
This coin is used therfore it might be difficult to see the differece.

but just dreaming:
There were a few versions of CM on this coins…
maybe once they forgot to punch CM while creating matrix?

Well it's worth to dream… :-)

but I'd prefer to see it with my eyes.

"grease filled die errors” were popular on Polish 1990-2010 coins
but you can always find “the soft surface” in tha area filled with grease.

In the case of over 1 000 000 000 of coins 10 000 errors might be hard to find.

On the other hand in Warsaw mint there was a time when workes produced rare coins -
putting  non standard planchets  and using original stamps…
