The token was added to mint sets 1996 KM#MS4.

The obverse was struck with the same die as used for the Obv. of the mint token put to sets KM#MS5 in 1997.
Mintage in Standard Catalog of World Coins is 15000, in local catalog Novotny V., Mince Сeskoslovenska, CR a SR - 2009, p.95 the mintage is 15000 -3500 =11500 pcs.
