In the center is the Coat of Arms of the Moscow Province and an inscription along the edge of the medal.
The coat of arms of the Moscow Province was approved in the 18th–19th centuries and replicated the main symbol of Moscow (Saint George), but in a simplified, "provincial" version. Shield, crown, and branches are a classic composition for provincial coats of arms.

The "Coats of Arms of Cities, Provinces, Regions, and Townships" series of medals included state coats of arms (small, medium, and large) of the Russian Empire, and regional symbols (coats of arms of the St. Petersburg, Moscow, and Penza provinces, the Kuban region, and others). Some of the pieces were issued by commission from Tarkhany Bank and Incombank, or as part of art projects.
The complete set of Coat of Arms of Moscow Region included 19 medals, and was distributed in a special gift box with CoA:
